Phase One

Preparation & Compilation

Reimbursements or out-of-network availments are permitted, regardless of whether the case is an emergency or not.

Outpatient Requirements

  • Claims Reimbursement Form (downloadable in LLIBI website: https://llibi.com/)
  • Medical Certificate
  • Valid Government ID
  • Consultation - Official receipts of consultation fees
  • Laboratory – Laboratory request form and official receipt of laboratory test
  • Medicine/ Vaccine – Prescription and official receipt of medicine / vaccine purchased

Inpatient Requirements

  • Official Receipts of Hospital Charges and Professional Fees
  • Detailed / itemized Statement of Account
Phase Two

Document Submission

Compile all the required documents and submit to your HR, Lacson account executive or through Online Reimbursement Portal available on our website under Member Health Services.

Final Phase

Approval & Payout

Reimbursement coverage is equivalent to 100% of the hospital bills or charges while professional fees of non-accredited doctors are reimbursed based on Lacson & Lacson's standard rates.

How to Avail Using Lacson Network

Step 01

Emergency

  • Present your Emergency Room card at the Emergency Room of any Lacson accredited hospital.
  • If you need to be confined, the Emergency Room card serves as the temporary LOA.
Step 02

Inpatient

  • Upon receipt of the admitting order from your doctor, request for an LOA from Lacson Client Care.
  • Present the LOA directly at the Admitting Section of the hospital.
  • Before discharge, see to it that PhilHealth is filed.
Step 03

Outpatient

  • Find out from the Lacson website (www.llibi.com) which accredited doctor you wish to consult with.
  • Call the doctor's clinic and request an appointment.
  • Request LOA to Lacson Client Care at least 24 hours before your scheduled appointment.
  • When you consult, present the LOA to the doctor's clinic.
  • If the doctor prescribes lab tests, the doctor fills out portion of the LOA for lab tests.
  • Present LOA to the HMO section for the approval code so the laboratory tests may be done.
  • Your OUTPATIENT LOA can also be requested through our Client Care Portal.
  • Scan the QR code to access the portal.
